The Director of Home Care reports to the Vice President of Senior Services and will provide strategic direction and oversight for Chicago Commons Home Care program, including services funded by government funders as well as private pay services.
Essential Duties & Responsibilities
- Supervise and coordinate planning related to operations of the Chicago Commons Home Care Program.
- Establishes mechanisms for ensuring high level of interaction, performance, and communication within the program.
- Overall supervision of the home care team. Conducts annual performance evaluations.
- Establishes and monitors annual program goals, a quality improvement plan, program budget and professional development resources for staff.
- Participates actively in agency Management Team and agency strategic planning.
- Monitors financial operation of program and works collaboratively with the Vice President of Senior Care to assess financial performance and make changes.
- Monitors and adjusts systems for measuring customer satisfaction and responsiveness to customer needs.
- Monitors and adjusts systems for assuring compliance with all funding sources and regulators.
- Uses data systems to support program management and to measure program effectiveness and makes recommendations for adjusting data systems as needed.
- Maintains good working relationship with governmental agencies and appropriate private funding sources.
- Advocates for quality senior care services in the community and establishes linkages with other providers and agencies.
- Responsible for implementing goals outlined in agency strategic plan.
- Responsible for overall supervision of the senior care team.
- Partners with the fund development team to seek out new funding.
- Partners with all departments of the agency to implement the strategic plan initiatives.
- Participates in the development of program budget.
- Other duties as assigned.
